What makes geriatric care different?

Services for older adults
We provide annual Medicare wellness visits, chronic disease management for diabetes, hypertension, heart disease, and arthritis, medication reconciliation, cognitive screening, fall risk assessment, advance care planning discussions, and coordination with specialists, physical therapy, and home health services when needed.
Coordinating your care
If you see a cardiologist, orthopedist, or other specialist, we make sure everyone is on the same page. We work to track your test results, coordinate specialist recommendations, and serve as your central point of contact. Family members are welcome at visits (with your permission) so everyone understands the plan.

We accept Medicare. Medicare covers an annual wellness visit for eligible patients, and our team verifies your coverage before your visit.
